Adriana (Giovanna Mezzogiorno), a medical examiner living in Naples, meets Andrea (Alessandro Borghi), an attractive younger man, at an occult ritual ceremony at her aunt's house. They engage in an intense, passionate one-night stand. They agree to meet the next day, but Andrea fails to appear. Shortly after, Adriana is horrified to find Andrea's body on her autopsy table, the victim of a violent murder with his eyes gouged out. As she investigates his death, she crosses paths with Luca, a man who looks identical to Andrea and claims to be his twin brother, drawing Adriana into an obsessive psychological maze filled with hidden family secrets and urban mysteries.
